Use this form to submit a Notice of Dispute as required by the “Dispute Resolution; Binding Arbitration; Class Action Waiver” section of the Mandated Reporter Training Terms of Use.

If you prefer to mail your dispute, download the MRT Notice of Dispute Claim Form, complete all sections, sign, and return to Simple Learning Systems at the address on the form. Simple Learning Systems and you will then negotiate in good faith for sixty (60) days before either party may initiate arbitration.
